To help quicken your first visit to our clinic, please print and fill out the appropriate form and bring it with you.  Click on adult or child new patient information form and the general patient information form.  Please read the 'Notice of Privacy Practice,' then print and sign the receipt of notice of privacy form.'  Bring these forms with you to your visit.  Thank you for your cooperation.
